Workers Compensation Claims Kit
OVERVIEW
The purpose of this guide is to ensure that all Team Members understand the process and requirements for filing a workers compensation claim in the event of a work-related injury or illness. Workers compensation provides necessary benefits to injured or ill employees, including medical expenses, lost wages, and rehabilitation costs.
PROCESS
- Eligibility and Reporting:
- All team members are eligible for workers compensation coverage.
- In the event of a work-related injury or illness, it is the responsibility of the Team Member to report the incident within 24 hours. The incident should be reported through the Squishable FreshService Help Desk (cud.li/Incident), to the team member's direct manager, and the location DM/RM or Department Manager.
- Failure to report an injury or illness in a timely manner may result in a denial of benefits.
- Workers Compensation Claims Kit:
- A Workers Compensation Claims Kit will be provided to any team member who receives medical attention for a work-related injury or illness.
- The Claims Kit includes necessary state-required documents provided by our insurance company.
- The Human Resources department may request completed documents from the Claims Kit to process the workers compensation insurance claim.
- Team members should keep the Claims Kit as a reference for the required materials and information.
- Document Submission and Processing:
- Team members should promptly complete the necessary documents in the Claims Kit as requested by the Human Resources department.
- All completed documents should be submitted to the designated Human Resources Team Member for processing.
- The Human Resources department will work with the insurance company to ensure timely processing of the workers compensation claim.
- Communication and Assistance:
- The Human Resources department will provide guidance and assistance to Team Members throughout the workers compensation claims process.
- Team members should direct any questions or concerns regarding workers compensation to the Human Resources department for clarification and support.
- Compliance and Confidentiality:
- Team members are expected to comply with all requirements and regulations related to workers compensation claims.
- The information provided in the claims process should be accurate and complete.
- All information and medical records related to workers compensation claims will be handled with strict confidentiality in accordance with applicable laws and regulations.
